Self-evaluation for worship team members

I'd like to try a self-evaluation form for my worship team members and myself. Have any of you out there ever done that/do you have a form you could share with me? I've been trying to create one from scratch and having a hard time...... thanks

We have our simple but stunningly profound (hahaha) form we make our vocalists fill out each ear now posted at:

this is NOT our comprehensive year end review where we all our participants evaluate the ministry. That is a different 4 step process we have (also on the site). This one is a self-evaluation form.
